Since I don't have a huge experience with PC98 themes, I don't have a worst yet.
But I know there is a best imo and that is Complete Darkness.

You need to listen to more. Only 4? Woah, there's so many worse themes you could be listening too!
As for worst theme, I'd say Bet on Death (Theme of Magic Stones) and Magic Formation (Theme of Sara).
These are the only themes I can legitimately say I dislike.

Finding The worst Touhou Music? This is more impossible than Kaguya's 5 requests!
But my least favorite pc-98 song is "eastern blue sky" from touhou 3. it's too simple, even for a ending...

I don't really know.
All I know is that the Touhou songs from SOEW and HRTP were bad. Maybe because ZUN was trying out a new PC-98 music maker or soomething.
And it's totally not Faint Dream ~ Inanimate Dream.. (Opinion, Duh!)

I would argue that HRtP has better music than SoEW. I'm serious about that. All of the songs in SoEW are just... meh, boring. Stage 1, 2, Marisa's theme, Stage 6, and Mima's theme are the only good themes I can hear.
Basically what I'm saying, SoEW has the worst soundtrack of all of PC-98, and Touhou in general.

Yep, i totally agree with you.SoEW might have a few masterpieces ,but all the other songs are bland and simple...IMO HRtP had the best PC-98 Soundtrack because all of the themes are nice to listen to and more memorable.Also, i think you meant Stage 5,because there is no stage 6 in SoEW  |
