You're playing MoF Stage 3. You hear "Gensokyo the Gods Loved", you see the title transition. You go through the horde of fairies, and then you get to the midboss. It should be Nitori right? WRONG. It's actually Sakuya. Wait a minute, Sakuya? Why is she here? You get her health low and she uses misdirection, you cap it. Go through the rest of MoF Stage 3, and you fight... Sakuya again. It's literally Sakuya's boss fight from EoSD, but in MoF Stage 3. This is mighty unusual. So you beat her. Go onto the next stage. It should be MoF Stage 4 right? WRONG. You've progressed to... DDC Stage 2???
That's where the Touhou Gauntlet idea comes in.
Basically, you go progress through random stages with random bosses. Depending on the mode you play, you get a different amount of stages and a different amount of randomness. It could be something like this...
Easy - 9 Stages. 9 Random Bosses for the stage. All bosses and stages are their easy mode variant.
Normal - 12 Stages. 12 Random Bosses for the stage. All bosses and stages are their normal mode variant.
Hard - 16 Stages. 16 Random Bosses for the stage. All bosses and stages are their hard mode variant.
Lunatic - 20 Stages. 20 Random Bosses for the stage. All bosses and stages are their Lunatic variant.
Extra - 7 Stages. 7 Random Bosses for the stage. All bosses and stages are from the Extra stages.
Random - 50 Stages. All bosses and stages have shuffled difficulties and variants.
Endless - Random Mode but endless.
Pulling bosses and stages through all of the windows danmaku games. Maybe even include spellcards from StB/DS/ISC. There could be a PC-98 mode too, though that would obviously only include SoEW, LLS, and MS. Hey, maybe there could boss battles made for the game characters that don't have a normal boss fight (phantasmagoria characters basically, and HRtP if you wanna go that far.)
How would the shot types be, and the game's mechanics? I have no idea, I've not thought this far.
